Plant Show Entry Form

Instructions
- Entry
forms will be available at show registration or may be printed in advance.
- When
printing the entry forms, set Page Scaling to None
in the Print dialog box.
This prevents the forms from shrinking on printers that do not print edge
to edge.
- NEW THIS YEAR:
Fill out two forms for each entry
- The Genus,
Species, and Exhibitor fields are mandatory. Please fill
them out completely.
- In the Genus
field write the genus, Dish, Terrarium, or Art, as
appropriate.
- In the Species
field write the species, cultivar, hybrid, or unknown hybrid,
as appropriate. If the entry is a dish, terrarium, or art, write
descriptive text.
- In the Exhibitor
field write your first and last name in order.
- The
optional Notes field is for
cultivation information: age, growing location, etc.
- Do
not guess at plant identification or spelling.
Research your plants before
the show. Print legibly.
- Do
not use defective forms. The entry form prints 8 per
page with 1/8" margins.
If your printer crops the edges or shrinks the forms you can pick up forms
at the show to fill out.
- Do
not fold the forms. The registrar will do this
after registration is complete to conceal
Exhibitor names for judging. Folding the form prematurely impedes
registration.
- Do
not write in the Entry Number field. If you
bring a plant for display only, and don’t want it judged, write NJ
in this field. Otherwise,
leave it blank.
